A system-wide effort to manage chronic diseases has earned recognition for physicians at Esse Health. The group was awarded the 2007 Primaris Physician Office Quality Award for the state of Missouri.

Esse’s Disease Management Team, comprised of physician leaders, nurse practitioners, a diabetic educator and other professionals with health care expertise, set the goal of providing appropriate care to chronic disease patients, using all means available. As simple as this may sound, Implementation can include surpassing many hurdles.  It includes the creation of population-based reports of patients with chronic diseases, alerts and reminders to prompt physicians to meet professional guidelines, the generation of patient-specific care plans, and the implementation of other process improvements such as group visits and intensive individual self management development.

Primaris Quality Partner
  Recognition from Primaris

Esse Health has been recognized as a Quality Partner by Primaris, the Medicare Quality Improvement Organization for the State of Missouri.

This two-year designation is a result of Esse's participation in the national Doctor's Office Quality Information Technology (DOQ-IT) initiative, sponsored by the Centers for Medicare and Medicaid Services.

DOQ-IT promotes the adoption of electronic health record systems and information technology in physician offices with a goal of enhancing access to patient information, decision support and reference data, as well as improving patient-clinician communications.
