New Patient Registration Doctor and patient
  When seeing a doctor for the first time, you must complete some general medical forms. Please go to the Office Locations section of this website and click on a specific office location to see any other forms that your provider may require you to fill out before arriving at your appointment. You will be asked to provide a current copy of your insurance card and social security number in order to create your unique medical record. Please arrive at the doctor's office approximately 30 minutes before your appointment to complete the necessary paperwork. Remember to make a note of any medications and/or supplements you are currently taking.  
     
  Registered Patients  
  Be sure to bring your insurance card to every doctor's appointment. Insurance companies change plan information and mailing addresses frequently. It is important we have current information to ensure your care is covered.  
   
Referral Requests  
If you have an insurance plan, primarily an HMO, that requires you to choose a Primary Care Physician (PCP), you will need a referral to receive services from a specialist. To prevent you from being billed, our referral staff carefully checks to assure that the services you need will be paid for by your insurance. We request that you contact the office at least 7 days in advance to assure adequate time to secure your referral. The specialist will need the referral prior to you seeing the doctor. We also need to have a copy of your current insurance card.  
   
All patient request forms that need to be completed and signed by a physician should be faxed or mailed with a self-addressed, stamped envelope. Since the physician is seeing patients throughout the day,
he/she is not able to address these requests during office hours.
