Breast Cancer Screening

This shows how well we help women screen for breast cancer by getting an image of their breasts (mammogram), as the National Quality Foundation suggests.

“Screenings” shows how many women 42-69 years old had a mammogram in the past two years.

Colorectal Cancer Screening/Prevention

This shows how well we help patients screen for and prevent cancer in the colon and rectum.

Cancer in the colon and rectum can be prevented if growths are found and removed early. A camera exam of the colon (colonoscopy or sigmoidoscopy) helps find growths. Cancer with no growths (silent cancer) can be found by a test for hidden (occult) blood in stool samples.

“Screenings” shows how many patients 50-75 years old had a colonoscopy in the past 10 years, a sigmoidoscopy in the past 5 years, or a stool test for occult blood in the past year.

Control of High Blood Pressure

This shows how well we helped patients with high blood pressure (hypertension) keep their blood pressure within the guidelines of the National Institutes of Health.

Esse Health was one (1) of only thirty (30) healthcare groups in the U.S. that received an award from the Centers for Disease Control for helping patients control their blood pressure.

Diabetes Care

This shows how well we help patients with diabetes control their blood sugar. A1c is a test for blood sugar. The lower the number, the better a patient’s blood sugar.

“A1c Done” shows how many patients 18-75 years old, and who have diabetes, had at least one A1c test in the last 12 months.
